New partnership to offer program to reach seniors
JEWISH FAMILY Service of MetroWest (JFS) has partnered with Northwest New Jersey Community Action Partnership to launch Senior Reach, to provide services to socially isolated seniors to combat declining mental health.
The program, to be implemented in Essex, Morris, and Sussex counties, offers community education and outreach to train individuals to identify at-risk seniors. Trained volunteers manage the Senior Reach call center referral line to screen older adults, determine their needs, and provide contact. Specialists make referrals to arrange for such services as connecting seniors in need with community resources or arranging for a home visit by a licensed social worker to determine the need for case management and mental health treatment.
When implemented in other areas, Senior Reach has shown a 93 percent engagement rate, with 87 percent of participants reporting a positive impact.
